What should an NDIS service agreement include?

A service agreement is a written record of what a participant and provider have agreed. The NDIA encourages one for most supports, especially Supported Independent Living, and it's required for Specialist Disability Accommodation. A good agreement covers:

  • the supports and how they'll be delivered
  • the price of each support
  • what the provider and participant are each responsible for
  • how long the agreement lasts and how it can be changed or ended
  • cancellations and travel
  • how to give feedback or make a complaint

Tips before you sign

  • Read it together. Ask the provider to explain anything you're not sure about, in a way that works for you.
  • Check the prices. Use the quote checker to compare the prices with the NDIS price limits.
  • Check cancellations. Make sure you understand how much notice you need to give and what you'll be charged.
  • Keep a copy. Both parties should keep a signed copy.
  • Get help if you need it. A family member, friend, support coordinator or advocate can help you.

Frequently asked questions

Do I have to have a service agreement?

For most supports it isn't required, but the NDIA recommends one so everyone knows what's been agreed. A written agreement is required for Specialist Disability Accommodation, and some providers ask for one for all supports.

Who should fill in the builder?

Either the participant or the provider can start it. Fill it in together if you can, then both read it before signing.

Can we change the wording?

Yes. You can choose which responsibilities to include and add anything else you've agreed. If you need something more detailed, get advice from a lawyer or advocate.

Is this a legal contract?

A signed service agreement can be a binding agreement. This builder gives you a plain-English template, but it isn't legal advice. Check it suits your situation before you sign.

Is what I enter saved?

No. The builder works in your browser. Nothing you type is saved or sent to Choosable, and it's cleared when you leave or refresh the page.

Ready to find a provider?
